Health Development Worker

The post holder will be responsible for the delivery and evaluation of a range of health and wellbeing initiatives across the Upper Springfield and Whiterock localities.

The Health Development Worker will work closely within the local community, community workers, partner organisations and other disciplines from across the community, voluntary and statutory sectors to:

  1. Deliver and evaluate an annual operational plan for the locality, which seeks to raise awareness and address the health & wellbeing needs of the population within the Upper Springfield and Whiterock, while taking into account the wider determinants of health inequalities.
  2. Deliver and evaluate a range of health & wellbeing related programmes targeting local residents and service providers.
  3. Deliver and evaluate a Social Prescribing Pilot Scheme.
  4. Facilitate the full and active participation of local people in the agenda to improve their health and well-being.
  5. Work within the community to undertake an on-going needs assessment to identify and prioritise the health and social needs of the population and to communicate these to relevant stakeholders.
  6. Support the design, development and delivery of local health development initiatives, projects and programmes that both address the health and social needs of our community.
  7. Support the capacity and skills of local people to become more actively involved in the development and delivery of health improvement programmes.
  8. Develop a detailed knowledge of the range of services available within the local community to support people and their family/carer from existing databases and available networks etc.
