The Cedar Foundation are Recruiting for the following
Transition Officer 21-028-TO-NHSCT-WEB
The Transition Officer will use an inter-agency approach to ensure a smooth transition post
school into education, employment as well as exploring health & well-being
opportunities in their local area.
Hours: 37 Hours per week
Contract: Temporary (Maternity Cover) approx 9 months
Location: Northern Health & Social Care Trust,
Base Location: 1A Woodside Road Industrial Estate, Ballymena, BT42 4QJ
Salary: Commencing on £23,080 per annum
Ideal candidates should possess the following requirements:
Essential
- RQF level 6 (e.g. Bachelor’s Degree) or equivalent level professional qualification in either Health & Social Care, Education, Training & Employment
- Minimum of 1 paid years’ experience in the direct provision of education, training or support
OR
- Minimum RQF 4 or equivalent in a relevant discipline`
- Minimum of 3 paid years’ experience as above
- Proficiency in IT skills, specifically Microsoft Word, Excel, Outlook
- Ability to work independently with good organisational, communication & interpersonal skills
- Hold a full current driving license valid for use in the UK and Ireland and have access to a car on appointment. This criterion will be waived in the case of a suitable applicant who has a disability which prohibits them from driving but who is able to organise suitable alternative arrangements in order to meet the requirements of the post in full.

- Experience & understanding of the barriers in relation to disability
- Previous experience of interagency working
- Facilitation and group work skills
