The successful candidate(s) will be expected to support people that are homeless, or at risk of becoming homeless through the impact of alcohol misuse.

MAJOR TASKS AND RESPONSIBILITIES:

 

  1. Assess on an individual basis the needs of each service user to ensure an active support plan is put in place with a lead agency.

 

  1. Develop effective liaison with other agencies ensuring appropriate pathways are established and agreed with accommodation providers, benefit agencies, drug and alcohol services/workers, etc to ensure best outcome for the service user.

 

  1. Contribute to effective team working and ensure professional joint working relationships with managers and staff from other agencies.

 

  1. Prepare, plan and deliver age appropriate information presentations and interactive workshops/activities to both large and small numbers of individuals in relation to the impact of alcohol.

 

  1. Prepare, plan and deliver appropriate diversionary activities to groups/individuals to those affected by alcohol misuse/addiction.

 

  1. Assist in the collection of statistical information on the work of First Housing Way2Go Project to reach deadlines as directed by the Project Co-ordinator.

 

  1. To use databases and other electronic management systems focused towards service provision improvements and quality outcomes.

 

  1. Have an absolute commitment to confidentiality.  Address all issues of this nature in a sensitive manner adhering to policies and procedures of First Housing.

 

  1. Participate in team and inter-service meetings.  Attend meetings as required.

 

  1. Encourage user participation in the development of the services.

 

  1. To cover seven days rota duties this includes day, evening and over night directed by the need of the clients to ensure effective service delivery.

 

  1. To participate in recording and reporting systems in regards to daily events in view of clear communication requirements.

 

  1. To develop and carry out needs assessment to address individual client needs.

 

  1. To maintain written case management files, support files and appropriate reporting systems.

 

  1. To carry out enhanced monitoring and evaluation of vulnerable individuals who have been identified as ‘at risk’.  (Risk Assessment) to ensure appropriate Health & Safety of users, staff members and organisation are not compromised.

 

  1. Staff employed in this service should have the capacity to understand the needs of this population and provide a flexible approach to this unique service.

 

  1. To attend training as identified and required in relation to personal and professional development.

 

  1. To prioritise ‘workloads’ and address and deliver a pro-active approach to address needs under pressure.

 

  1. In conjunction with team colleagues to transport and assist the client to access accommodation and other related support services.

 

  1. Any other duties appropriate to the post
