Integrated Care Partnerships (ICPs) are playing a key role in how health and social care services in Northern Ireland are being transformed. They are helping to change the way care is delivered. There are a total of 17 ICPs, collaborative networks of care providers, bringing together healthcare professionals (including doctors, nurses, pharmacists, social workers, and hospital specialists); the voluntary and community sectors; local council representatives; and service users and carers, to design and coordinate the delivery of local health and social care services.
These Partnerships were initially established in 2013 with short term funding but are now being mainstreamed and funded recurrently. Integrated Care Partnerships are key to delivering transformation as outlined in Delivering Together.
The role of the ICP Third Sector Co-ordinator will be to help build a strong vision for the third sector’s role within ICPs, to operationally support third sector representatives within ICPs and to work across the system in an outreach role to promote integrated pathways of care which optimise the role of the Third sector. The ICP Third Sector Co-Ordinator will work closely with the Programme Director Integrated Care, the local ICP clinical and business support teams and the ICP third sector representatives, to actively lead and develop the role of the third sector in health and social care delivery through ICPs.
