You will make a difference to clients and their families by supporting them in achieving outcomes which will enhance their quality of life and independence.
You will be providing a needs-based support service to people affected by stroke which will include:
- Developing referral pathways and establishing new service initiatives;
- Pro-actively maintaining existing networks and partnerships;
- Receiving referrals from a variety of sources;
- Delivering Stroke Family Support Service in the Mid Ulster Area by carrying out client needs assessments via home visits;
- Delivering Weekly Stroke Activity Group sessions to Stroke Survivors (in the Causeway and Mid Ulster Areas)
- Delivering NICHS Taking Control Programme
