Location: Up to 25 mile radius from home
Reports to: Hospice at Home Co-ordinator
Band: Based on Band 3 (£17,125 pa pro-rata/ £8.75 per hour)
Hours: Bank
Role
To provide support and respite care for patients and families in their own home. To deliver high quality nursing care to patients with palliative care needs by implementing and evaluating individualised care plans.
