Background to the project:
The project offers an intensive employability mentoring support service for young people leaving care in Belfast. This is offered for clients aged 16 - 17 (alongside the LAC Teams) and aged 18 – 21 (alongside the Leaving Care / After Care Team).
The project involves a 12-week mentoring support programme for young people who are looked after or who have a care history within the Belfast Health and Social Care Trust.
The programme is primarily targeted around education, training and employment.
Background to the role:
The aim of the role is to work alongside the Assistant Manager in the delivery of the service. This will include developing a wide range of services and opportunities to young people who present with varying levels of need. This will be achieved by bringing together the skills and experience of a multi-disciplinary network of staff and agencies.
Main Responsibilities:
- Provide support and interventions in order to care for experienced young people.
- Manage a caseload of clients from social services, with regular case review.
- Carry out detailed assessments of clients’ needs and support them appropriately, using validated tools prescribed by Start360 and all of the expertise available within Start360’s other services.
- Develop care plans in agreement with social services and each client, based on identified need.
- Deliver therapeutic group work and support programmes alongside other members of the team.
- Ensure that the health, safety and welfare of all clients is paramount at all times.
- Work in collaboration with other team members and social services, to ensure a seamless service for young people and their families.
- Develop innovative ways of working with young people and support the individual intervention of each client.
- Develop and maintain professional and effective relationships with all stakeholders: young people, families, and relevant statutory, community and voluntary organisations.
- Develop processes through which young people can inform and shape the development of the Service.
- Record all client work on Start360’s Client Information System.
- Provide monthly statistics and progress reports as required by the Belfast Health and Social Care Trust.
- Attend 4-6 weekly reviews and annual appraisals with the Assistant Manager.
- Support the Employability Service through workplace tours, application sessions, annual job fairs and an annual celebration event.
- Participate in team and organisational meetings, training, conferences and maintain own professional development.
- Assist in maintaining a relaxed and friendly atmosphere, in order to enhance the quality of care provided to each client and maximise their opportunities for choice.
- Adhere to Start360’s standard operating procedures at all times; in particular Health and Safety, Child and Vulnerable Adult Protection, Lone Working, Equal Opportunities, Data Protection and Confidentiality.
- Contribute to the overall evaluation of the project.
- Carry out other responsibilities that may be required, as commensurate with the post.
