Working closely with local Respiratory teams, as well as NICHS colleagues, you will make a difference to clients and their families by supporting them in achieving outcomes which will enhance their quality of life and independence.

Working closely with local Respiratory teams, as well as NICHS colleagues, you wil make a difference to clients and their families by supporting them in achieving outcomes which will enhance their quality of life and independence.

You will be providing a needs based support service to people affected by chronic lung conditions which will include:

  • Developing referral pathways and establishing new services
  • Pro-actively maintaining existing networks and partnerships;
  • Receiving referrals from a variety of sources
  • Carrying out client needs assessments via home visits
  • Delivering NICHS Taking Control Programme and Breathing Better Support Groups and networks

 

Main Responsibilities

Service Delivery

  1. To support the implementation of the NICHS Impact and  Outcomes Framework and to contribute to is continuing development to meet the needs of people affected by chronic lung conditions;
  2. To receive, assess and action referrals against established criteria;
  3. To undertake assessments and set goals with service users using standardised tools;
  4. To provide listening, understanding, advice, information and support to service users;
  5. To establish a strong network base, including referral pathways,  and refer onto other services as appropriate(both internal and external);
  6. To deliver services in the community as per NICHS Service Descriptions and Standard Operating Procedures;
  7. To plan and deliver all programme activity sessions in line with NICHS policies and procedures;
  8. To encourage an ethos of self-management and empowerment, and where appropriate to act as advocate for the client and to ensure that client opinions are sought regularly on the services provided;
  9. To provide cover for other NICHS service user groups and services when required.

General

  1. To prepare reports as required for the management team of NICHS and appropriate Health and Social Care Trusts and other funders;
  2. To maintain service user records and files as required, in line with NICHS policies and procedures;
  3. To input into the maintenance of NICHS’s Client Management System;
  4. To set goals with service users, maintain individual progress reports for each client and organise reviews accordingly;
  5. To assist in the development of service budgets while ensuring that financial procedures are adhered to and comply with NICHS accounting procedures;
  6. To ensure that confidentiality of information is maintained at all times;
  7. To identify, and make use of, any opportunities to publicise NICHS and its services and support NICHS Communication and Engagement Strategies;
  8. To contribute to and be involved in campaigns to raise awareness of Chest, Heart and Stroke illnesses;
  9. Any other duties as requested by the Head of Service or Director of Care which fall within the scope of the post.

 

Recruitment, Selection and Management of  Volunteers

  1. In conjunction with the Head of Volunteering,  recruit, select, induct and train a pool of volunteers to assist in the provision of services to service users;
  2. To supervise and support the volunteers in all matters relating to the services provided;
  3. To maintain volunteer records including attendance at training and development sessions;
  4. To ensure all volunteer expenses are accurate and claimed on a monthly basis within the accounting procedures timeframe.

Please note:

This post falls within ongoing NICHS Service development consequently there may be changes and amendments to some elements of the role and subsequent duties and responsibilities.

The post will require you to deliver Be-spoke Support Programmes to clients and you will be required to undertake the appropriate training as part of this role
