Grant Funding Opportunity - Social Prescribing Coordinator Pilot Programme

This programme will be piloted from 1 March 2015 until 31 March 2016 to include two GP practice areas (one rural and one urban) within the legacy council areas of Derry City, Limavady, Strabane, Omagh and Fermanagh.
