Senior Project Worker (Maternity Cover)

To assist in the management of the delivery of a floating support service, addressing the housing support needs of families and single people over 25 within the Southern area.

Key Responsibilities and Accountabilities 

SERVICE DELIVERY

  • To assess suitability of referrals to the criteria and objectives of the service. 
  • To establish supportive relationships with service users and deal with  difficult and/or problematic situations in an appropriate and sensitive manner, keeping within the boundaries of policies and procedures and recording incidents appropriately.
  • To take on case manager responsibilities for identified service users. This will include the compilation of individual support plans with the service user to achieve set goals. 
  • To work with the project worker to implement action plans by providing the appropriate information, opportunity and support.
  • To ensure all service users are positively involved with their own development.
  • To liaise and advocate with outside agencies, e.g. NIHE, Benefits Agency, and other statutory and voluntary bodies as appropriate.
  • To establish and maintain good working relationships.
  • To be involved with client participation, being innovative and motivating others in this area and be responsible for a designated field of work. 

HEALTH AND SAFETY

  • To ensure that Homecare’s Health & Safety policies & procedures are adhered     to at all times, including Lone Working safeguards.         

STAFFING RESPONSIBILITIES

  • To provide support and supervision to designated project workers, Identifying training and development needs.
  • In conjunction with the Support Services Manager, develop induction programmes for each new member of staff.
  • To take part in the recruitment and interviewing of new staff where appropriate.
  • To carry out supervision with designated project workers in accordance with Homecare’s policy.
  • To maintain appropriate staff records.
  • To ensure that project workers are keeping their records and all other information, including monitoring forms up to date.  

ADMINISTRATION

  • To maintain written records including service users files and any monitoring information as appropriate, input and retrieve data on the monitoring form. 
  • To ensure clear and concise records of all supervision sessions are kept and regular reviews of action plans are carried out and changes recorded.
  • To be involved with the Support Services Manager when required  in the     preparation of reports, statistics and other papers that may be required by the  General Manager, Director or Supporting People. 
  • To ensure that any complaints are recorded and dealt with in an appropriate and        timely manner. 
  • To assist the Support Services Manager if required in using the SPOCC net      system to report back to Supporting People. 
  • To ensure that all staff are using the Outcome Star as a way of measuring   outcomes and recording this information accordingly.   
  • To audit files when required and report back any issues or concerns to the Support Services Manager
  • Any other administrative tasks as may be required  to ensure the smooth running of the project. 

GENERAL RESPONSIBILITIES

  • In conjunction with Support Services Manager to continuously develop the role to ensure that all tasks are being undertaken in an effective and appropriate manner which meets the strategic aims and objectives of Homecare Ind. Living.   
  • To support the Support Services Manager in the effective management of the project.
  • To participate in team meetings and to ensure an agenda is prepared and minutes are taken. 
  • To participate in internal/external meetings as required, and attend training events, conferences and other functions if required. 
  • To participate in regular supervision and annual appraisal, and help in identifying your own job-related development and training needs.
  • To contribute to, and be supportive of, service user involvement in our service and in all levels of decision making throughout the organisation.
  • To ensure that all Homecare’s policies and procedures are being adhered to, particularly those relating to Health and Safety, Code of Practice and Confidentiality.
  • To contribute to the effective implementation of Homecare’s Diversity and Equality Policy.
  • To at all times undertake your role in a professional manner maintaining a high quality standard of work, and  to always work in accordance with the aims, values and ethos of Homecare Independent  Living
  • Undertake any other duties that may be required which are commensurate. 

SPECIAL CONDITIONS

  • To carry out other duties and responsibilities commensurate with the post and the needs of the organisation in order to develop and maintain service delivery. 
  • The post requires the holder to have a clear disclosure check in accordance with the regulatory body Access NI/Garda Clearance.
  • You may be required at times to transfer (depending on business needs) to alternative departments/areas.
