To support people in the criminal justice system disproportionately represented as having significant mental health problems. Addressing their needs & supporting integration,the project will see improved mental health & a reduction in offending behaviour.
To carry out assessments of reintegration needs in respect of people referred to the project, prepare support plans and carry out reviews.
Deliver an individual, needs-led approach which supports reintegration, recovery and reduced risk of reoffending.
To liaise with referral agencies, establish and maintain protocol arrangements for information exchange.
To work in co-operation with key agencies involved in providing services to support integration and social inclusion.
Promote the project with key stakeholders and contribute to formal meetings with them to review the work of the project
Maintain case records, data collection arrangements and produce project reports as required. To support policy comment on the related issues
Contribute to the development of the project and the range of interventions available.
