To be responsible for the day to day clinical running of continuing care service users in their area.

 

To be responsible for the day to day clinical running of continuing care service users in their area. To ensure that all staff supervision and professional development needs are met.  Work as an effective team member by taking responsibility for assessing, planning, implementing and evaluating the day to day operational & clinical care provided to service users with complex health needs.


Key Responsibilities and Accountabilities:

 

Clinical, Compliance and Training

 

  • Actively support research and audit.

 

  • In conjunction with NHS Care Managers. to review and update case managed continuing care plans

 

  • Identify suitable Care staff and organise relevant training courses.

 

  • Assist with training for all specialist training needs ( within skill set)

 

  • Clinically supervise and appraise staff involved in  CC packages only

 

  • Maintain and evidence PREP ( Post Registration Education Portfolio)

 

  • Provide a high standard of evidence based care to service users in line with Homecare Independent Living Policy & Procedures.

 

  • Have day to day responsibility for assessing, planning, implementing and evaluating standards of care provided in the community by Clinical Support Workers.

 

  • Support the Regional Case Manager in the service delivery of continuing healthcare.

 

  • Ensure own knowledge, skills and expertise are up to date.

 

  • Ensure new and junior staff is assessed as competent before undertaking enhanced roles eg. oral drug administration, PEG feeding, suctioning,etc.

  • Treat with complete confidentiality, all service user information you have knowledge of.

 

  • To report any area of concern to the Quality Manager.

 

  • The job description will be reviewed annually and may be amended after discussion with the post holder.

 

  • This job description is intended as a guide to the main responsibilities of the post, and the post holder will be required to undertake such duties appropriate to the position as may be required by the Manager to ensure the smooth running of the service.

 

Operational Management and Business Development

 

  • All service user safety incidents to be escalated as per Company Risk Management Policy & Strategy

 

  • To have a weekly Conference call with Line Manager for the purpose of updating and attendance at a monthly meeting thereafter.

 

  • To work shift patterns/unsocial hours as per the clinical needs of the packages

 

  • Demonstrate the ability to manage the case load effectively, dealing with unexpected issues eg. Sickness.

 

  • Delegate staff in conjunction with Regional Case Manager according to skill mix, work load and service user dependency.

 

Customer Service, Team working Planning and organising

 

  • Effectively deal with unplanned events which may impact on the smooth running of the shift

 

  • Demonstrate the ability to manage own and others workload effectively.

 

  • Communicate appropriately with a range of people in a manner that is consistent with relevant legislation, company policies and procedures.

 

  • Work co-operatively within a team and ensure effective   communication is actively encouraged between all disciplines.

 

  • Keep accurate and complete records consistent with legislation, Policies and procedures in line with the NMC Code of Conduct.

 

Health and Safety

 

Within their own area of responsibility, the post holder will be responsible for assessing the risks to the health and safety of staff, service users and visitors or to the achievement of the Company’s objectives.  The post holder must ensure that identified risks are reported so that significant clinical and non-clinical risks are included in the Company Risk Register.  The post holder will participate, as required, in clinical and non-clinical audit projects aimed at improving patient care and the effective and efficient delivery of service.

 

Clinical Governance

 

  • Have an awareness of Clinical Governance and its effect on practice.
