The Transition service supports young school leavers with learning difficulties or autism and their parents/carers to explore future options and gain independence through training, mentoring and sampling work placements. We help them to start planning early and help take the anxiety out of taking that big next step from school into the world. It's important families understand their options so we provide work visits, work placements, and accredited training, helping shape a package of support for parents/carers and each young person leaving school.
