Job Purpose:
To provide short-term support to those people deemed fit for discharge from hospital who live alone or do not have sufficient support in order to help them rehabilitate.
Essential Criteria:
- At least 6 months paid / unpaid experience in a caring role.
- Ability to communicate effectively both verbally and in writing.
- Commitment to promoting and protecting the rights of vulnerable adults.
- Access to transport in order to carry out the duties of the post.
Desirable Criteria:
- GCSE (or equivalent) standard of education.
- Relevant qualification e.g. QCF Diploma in Health and Social Care, Level 2 or equivalent.
