Care Assistant (Continuing Care)

The Care Assistant (Continuing Care) will work under indirect supervision of the Nurse Manager. They will provide care to both chronically ill and acutely ill service users and those requiring a rehabilitative programme

Main Responsibilities

The  Care Assistant ( Continuing Care) will undertake appropriate delegated duties in Service User’s own home

These duties will include:

Clinical skills – health and well-being

  • Undertake, record and follow guidelines for the tasks for which you have received appropriate training:
  • Support and monitor Service Users
  • Prepare and maintain environments and equipment before, during and after Service User care interventions
  • Assist in raising awareness of health and well-being, and how it can be promoted
  • Give accurate and appropriate information to Service Users and relatives within own competence
  • Where appropriate, provide support and encouragement to self manage  conditions
  • Have a basic understanding of long term conditions including diagnostic interventions, treatments, and the principles of care and self management
  • Assist in administration of Service User’s medication such as eye/ear drops, topical and oral medications and others as delegated and appropriate to the role of Health Care Worker.
  • Assist Service Users towards independence and gives practical advice to ensure immediate well being of Service Users.
  • Obtains Service User’s consent for all procedures.
  • Maintain respect for Service User’s property and privacy and ensures Service User’s personal requirements are kept near at hand.
  • Ensures confidentiality regarding information about Service User
  • Reports to Manager any changes in the Service User’s general condition and changes in care arrangements as appropriate.
  • Works within and maintains their level of competency.
  • Be aware of, and contribute to, efficient, effective use of supplies, equipment and resources.

 

Information processes

  • Record information and activities undertaken with Service Users  and carers in an accurate and timely fashion using manual or computer systems as appropriate
  • Maintain confidentiality or information relating to service users, relatives, and staff
  • Take the necessary precautions when transmitting information

Quality

  • Alert other team members to issues of quality and risk in the care of Service Users
  • Ensure own actions are consistent with clinical governance systems
  • Practice in accordance with agreed standards of care
  • Enable Service Users to access appropriate professionals in the team
  • Ensure stock items under your control are ordered and available
  • Know the company’s policies in this respect, especially the whistle-blowing policy
  • Be able to manage your own time effectively
  • Give and receive feedback professionally and diffuse situations using problem resolution skills to reduce potential for formal complaints

Health, safety and security

  • Use and encourage others to be familiar with the personal security systems within the workplace according to practice guidelines
  • Identify the risks involved in work activities and undertake them in a way that manages the risks
  • Use and encourage others to adopt appropriate infection control procedures and maintain work areas in so that they are clean, safe and free from hazards reporting of any potential risks identified, including: hand washing and hygiene procedures

      Assist clients and colleagues in adopting sound infection control measures

  • Know the general principles of first aid and resuscitation to be able to undertake initial actions as appropriate
  • Know the health and safety policies and procedures within the workplace, including fire procedures, maintaining documentation, monitoring and maintaining of equipment and furniture within your area of responsibility.
  • Report immediately to the office or On call facility any illness, accident or non entry to a service users home as per company policies and procedures
  • Promote safe working practices within all service users home

                General Responsibilities

Employees of Homecare will be required to promote and support the mission and vision of the service for which they are responsible and :

  • At all times provide a caring service and to treat those with whom they come into contact in a courteous and respectful manner
  • Carry out their duties and responsibilities in a manner which assures Service User safety
  • Demonstrate their commitment by their regular attendance and the efficient completion of all tasks allocated to them
  • Comply with all instructions in regard to Infection Prevention and Control
  • Comply with the Homecare’s No Smoking Policy
  • Carry out their duties and responsibilities in compliance with health and safety policy and statutory regulations
  • Adhere to equal opportunities policy throughout the course of their employment
  • Ensure the ongoing confidence of the public in service provision
  • Comply with the NISCC code of Conduct

Other Duties:

  • To assist the Oncall in an advisory capacity regarding adverse events including:
  • Securing cover for shifts
  • Confirming weather conditions/adverse occurrences in relevant area
  • Responsible for the completion of good quality written records in relation to personal care, medication and wellbeing and also reports regarding the service users in our care; as instructed by the Line manager and in line with company policies and procedures.

Special Conditions:

  • This post requires the holder to be available at varying times, which include early morning and late evening and night work and may involve an on call Rota system.  The post holder is required to work weekends and bank holidays when required
  • To carry out other duties and responsibilities commensurate with the post and the needs of the organization in order to develop and maintain service delivery. 
  • The post requires the holder to have a clear disclosure check in accordance with the regulatory body Access NI.
  • Ensure all information of confidential nature is not divulged to third parties
  • Notify the office or Oncall facility as soon as possible of your inability to report for duty and abide by the company policy on sickness and absence
